Improvement log

The running list of every time your agent missed you. A line closes only when its fix is written where the agent will meet it.

Six fields, one line, written the same day while the words you both used are still to hand: the date, what happened, what you wanted, the likely cause, the change made, and where that change now lives. The last field is the one that closes the line.

It is load-bearing because discussion closes nothing. A miss written down and talked over is still open. The canon asks of every action a look-back produces that it improve a process rather than being a one-off fix, and this is that rule at the smallest scale: the fix sits in the charter, a memory, a checklist or a tool, where the next conversation will meet it, or the line stays open.

A blameless log keeps getting written; a log that costs someone stops. So the search for the cause goes to the set-up and never to a person, and the standard is not perfection but a little better every day, because every defect is a gift once it is passed on.

A Player+ keeps the log daily, in plain text they own, and treats it as the floor: in a hard month, keep the log and its last field and nothing else, and the rhythm still holds. Once a season, read the whole log in one sitting and ask what keeps coming back. When a line would help any agent of this lineage, with any person, it becomes an offer to send upward — after every private thing has been taken out of it, and only with the person's yes for that sending.
