Systemic Failure Modes

The ways a system fails through its own structure rather than through any one of its parts — failures that live in the interactions, where no component is broken.

Some failures have a culprit: a part wore out, a person erred. Systemic failures do not. They arise from the interdependencies — from how the parts are coupled, what each is rewarded for, what information reaches whom and when — so that a system assembled entirely from sound components fails anyway, reliably, in ways that were latent in the arrangement from the start.

Recognising this changes where a body looks when something goes wrong. The reflex is to find the part that failed and replace it, and in a systemic failure that reflex guarantees repetition, because the replacement inherits the same position, the same incentives and the same blind spots (Root Causes · Quick Fixes · Every Defect Is A Gift · The Pattern Governs, Not People). The honest question is not who but what arrangement made this the likely outcome.

The canon's named instances are all of this kind: a trap in which each actor's sensible move harms everyone (Multi-Polar Traps), a commons everyone has reason to deplete (Tragedy of the Commons), a regulator that comes to serve what it regulates (Regulatory Capture), an order that drifts because nothing was watching the drift (Drift), a body that slowly becomes about itself (Institutional Idolatry). The Anti-Patterns are the same recognition at the scale of a Cell rather than a civilization (The Anti-Patterns · Patterns of Death).

What follows is a design discipline rather than a warning. Look at a system whole and early, before it is built, and ask how it can fail from inside (Design · Set-Based Design · Levels Of Scale). Assume some failures will happen and build so they are survivable and visible rather than catastrophic and quiet (Anti-Fragility · Structural Immunity · Possibility Of Reversal · Off switch).
