Scribe
Whoever captures, in the moment, what was decided and what was learned.
The scribe writes while the gathering is still in the room. Four things are caught: what each person said they will do and by when; the gap between last week's word and what actually happened, with the line learned from it; what was decided and the reason for it; and anything that blocks, with an owner and a date (Commitment, The gap, Decision Records, Issue). Written later, from memory, these become approximately true, which is the same as useless.
It is a distinct hand from the Keeper, who holds the record between gatherings, and from the Host, who is keeping the room and cannot also be watching the page. In a small Cell one person may carry two of the three; naming them separately is what lets a body notice when nobody is actually doing one of them.
The discipline is plain text a member can read and correct. From every gathering, a record; a decision log that carries the why so that settled things are not re-fought by accident; a defect log that takes every miss the day it happens and closes a line only when its fix sits where the work will meet it (Every Defect Is A Gift). What is owed is then computed from the record rather than remembered (The Record).
